An assessment aims to make sure that you get the right kind of help with your health and social care needs at the right time, whether you are in hospital or at home. Leeds’ ‘Single Assessment Process’ means that both your health and social care needs are looked at at the same time. It can be carried out by a social worker, district nurse or other healthcare professional.
If you would like an assessment, call the Contact Centre on 0113 222 4401 or speak to your GP.
You will need to have an assessment before you can get any help with care from Leeds City Council Social Care. Unfortunately, not everyone is eligible for help with their care; Leeds Adult Social Care uses ‘eligibility criteria’ to decide who should get help with their care.
You may have to pay towards the services you receive and will be assessed financially.
