Description
Co-ordinates care for people aged 65 and over with a mixture of health and social care problems, including difficulties in managing daily life. The aim is to help people live as independently and safely as possible. Work includes supporting people discharged from hospital, co-ordinating help to prevent people from having to go into hospital unnecessarily and supporting people at immediate risk where there has been a sudden change in their family's or carer's circumstances.
